Tylenol and any other drug, prescribed or over the counter, that can cause liver damage in "normal" (that is, people who have not had weight loss surgery) who take large amounts, can cause problems for gastric bypass patients in small amounts. Any drug that is metabolized by the liver can cause harm to the liver in a person who is losing or has lost a massive amount of weight. Fatty liver, or Non-Alcoholic Steatotic Hepatitis (NASH), is quite common in obese people. Whether you are losing weight rapidly from a diet like Atkins, Optifast, or from weight loss surgery, your liver is fragile. Mainly because of all the fatty material being metabolized there. If you throw in medicines or alcohol into the mix, the risk of liver damage climbs.
