Please Help!

Hi everyone, I thought that I posted this yesterday but I don't see it on the board, anyway I am getting my paperwork together and my PCP needs a detailed diet plan to send in to the insurance co. does anyone know exactly what they need (AETNA POSII) I mean I have tried everything out there twice for the past 15 years so can anyone help me I mean is there a certain format or chart that it needs to be in? Kathina, all I did is make a list types of diets lbs lost what year how long you tried it know my pcp doctor typed it up for me and signed off on it for me so hopefully so will yours if you need more information let me know what surgeon are you using

Thanks so much Dawn, I am looking to use Dr. Brient in Gainesville Florida because I looked all through the surgeons in Georgia and most of them would not accept my insurance except for the ones in Atlanta and that is a good 4 hrs from me (Valdosta Ga), I just can't see driving that far with all of the appointments that I will have to go both pre-op and post op! I will call my PCP's office in th am and see if they will type it for me if I just send in a written form, thanks again and God Bless you! Katina Rye

I have Aetna and they want a six month physician supervised diet or a 3 month surgical multidisiplinary diet. I did the six month. Email me at [email protected] and I will send you what I sent in and got approved on the first try. Susan

Hi Katina, I just listed alllllllll the wonderfully unsuccessful diets I had, and gave an approximation of when I did it and for how long.... It's not like they are going to go back and investigate or anything! LOL! GOOD LUCK Tami
