Can anyone that was recently approved by BCBS (HMO), please advise me what type of documentation is needed for the attempts at weight loss. Does it have to be supervised?

Hi Nicole I just got approved through BCBS-HMO and they really just want something from your doctor say you were coming to the doctor and everytime you came this is what you weighed. My doctor just documented that i was 350+ on every visit. It worked for me and I hope I have been some assistance to you

Hi Nicole, I too was approved by BCBS HMO, I was required to have a 6 month diet history, a 10 year weight history, a letter of medical clearance and medical neccesity, there was also several blood tests that they required. Warning though, if I would not have kept calling them I would have known that I was approved until I got the approval letter (the actual approval only took about a week or so) The letter was dated early April, I didnt receive it until early May and only had until July 5th to have surgery..be on top with them and who ever you are having surgery with...Hope this helps Erica
