Help! Letter needed for BL & AL

Help! My plastic surgeon Dr. Wang is going to submit for my breast lift and arm lift next week. She did my TT and got an approval right away. I know my insurance does approve for this procedure so I am hopeful but dont know where to start. Becuz I dont want to be denied then do an appeal letter I want to send for approval with a letter from me in the beginning. Something I had not had to do before. So Dr. Wang wants me to write a letter for faster approval. Now I dont know what to write can you help me? I dont know how to begin. I know there are things they are looking for and certain things they want to hear. I know I need to emphasize somethings and perhaps downplay others. Can anyone help me with a form letter so I can have an idea that worked for them. Plz email me I would greatly appreciate it. The best thing that I can suggest is submitting photos of you before your surgery, while you were losing weight, and presently. Also, if you have your measurements of before surgery and presently. Hope that this helps!! Norma
