Need help understanding the process

I called today and they told me they absolutely do not cover weight loss surgery. I asked if there was some type of appeals process or anything I can do. He gave me Care Coordination and they gave me a fax number for the doctor to send a letter stating the surgery is medically necesary and the telephone number to followup the request. My question is I've been reading alot about having to do a 6 month supervised diet, see a nutritionist and pysch eval. Is this stuff I do in order to support the letter my pcp sends to Care Coordination to establish medical necessity or are these steps done after the pcp sends his letter? Thanks.

It depends on the insurance company. Mine covered it only if medically necessary and they had specific criteria I had to meet to be medically necessary which was the 6 mo diet thing, nurtritionist and psy eval. Be sure to read my profile and see if that helps answer some of the questions as well.
