Sounds like you guys had too much fun this weekend! I hate missing a good time! Maybe next time.
I have decided on Dr. Chris Hart. I called his office to get a consult and have to call back Monday to answer a 20 minute questionaire. My PCP has given me the go, and now for all the tests! I am really excited, and eager to get this going. I called BCBS and they are still paying for this surgery, even though I had heard different. YIPPEE!
My main worry is about the weight loss history! I have never stuck to a weight loss plan for very long. I did do the Phen-phen thing for almost a year, and had very good results, only to gain it all back and more. I have no documentation, the physician is out of business, and I can't get the records. If anyone has a template or something they can e-mail me concerning this, please send it to me at tootiemay@aol. I will you for ever! Maybe it will relieve my mind.

Hi Karen! I have BCBS as well, and did not have any documentation from any source other than my memory. All I did was to list the following: the TYPE of diet or program I was on (WW, the Zone, Carb Addicts Diet, Atkins, Phen-fen, Diet Center, etc., etc.), the YEAR I went on each diet, the [approx] number of months, and the [approx] amt of wt. lost on each. I put them in roughly chronological order, so they could see that I tried several of them more than once. The Phen-fen thing was 8 or 9 years ago, which was the only doctor-supervised diet, and that was a concern to me too. But what I listed was enough for them, since they approved me on my first letter. I didn't use BCBS, but I did use Dr. Hart. He's great!! As to the diet history, put down as much as you remember about dates, what you lost and how much you gained back after. I didn't have a lot of documentation on mine either. I think I just put it in an Excel spreadsheet. Good luck and keep us posted.

I went to my consult appointment with Dr. Hart today...he is wonderful as is his staff. Mildred is absolutely adorable and Dr. Hart is very thorough and has wonderful bedside manners. As for the weight loss history, you dont have to have written papers from the dr office, it is all memory, and BCBS does not require you to have copies of medical records....woohoo. I go next week for my testing whi*****ludes lung function tests, blood work, upper GI series xray, and a stress test. I was able to schedule my dietary consult on the same day I am going to be in Atlanta for my testing. It takes me 3 hours to get there from my house so I try to get it all done at once. The only thing I have left to schedule is the psychiatric consult, which I have to wait on because the offices were closed today I guess since I got the answering machines when I called. After that, all will be set to send into the insurance and hopefully will have a date soon. I will keep you posted, and good luck.. I will look for your post soon.
