I have this insurance through State Merit. I know of several people who have been approved, with usually no trouble.
I am just getting started, but this is what I found out so far.
You will need:
-a thyroid profile (your PCP can do this in the office)
-a letter of medical necessity from your PCP
-a letter of surgical clearance from your PCP (this can be in with the other letter.) It will include any test your PCP wants you to have.
-3 years weight history
-5-6 nonconsecutive months of SUPERVISED diets in the last 1 to 5 years. This includes anything your doctor has told you to do with dieting, like prescriptions, atkins, WW, etc. Also it can include Jenny Craig, etc. It just has to be given by a doctor or registered nutritionist. I got a copy of my records, and went through them all. Any time the doctor mentioned trying something new, I used that. Like, one time, he suggested the Atkins. I did that for two months.
Also, at least 100 lbs overweight. A BMI of 35 of more with co-morbidities, or 40 without them.
I think that is it, or at least all I can remember. I am going ahead and getting a sleep study, since I know my surgeon will require that. I would say, get your PCP to do the blood work, run an EKG, and get any tests he/she wants you to have to clear you. Also, start gathering your medical records from all doctors to get your dieting history.
I hope this helps! I will be thinking of you. I hate the hurry up and wait game. Since I have decided to do this, I am terrified they will quit paying for it before I am approved. Hang in there. My friends were approved within 4 to 5 months of the time they started everything.
