Weightloss Attempts History for BCBS
I'm just starting to work on collecting info for my insurance packet and I'm trying to fill out the form for physician supervised weightloss attempts in the past 5 years. Here's the problem, the past five years I've been doing it on my own...since I realized the only thing I was losing was my money. In the past (more than 5 years) I've done TOPS, Physician's Weightloss Center, Meridia, Fastin, etc. But in the past 5 years - nada. Well, actually, I have been on Wellbutrin for about 2 years. But that was only partly for weightloss and in my file it will not state "for weightloss" because if it had the insurance would have not paid for it! I have had one presciption for Phentermine (1 month) and the doctor advised me to stick to the Sugar Buster's diet (which I did for about 3 months). But I don't know if this was documented in his notes or not. What do I do? Do I list the old stuff? Do I start something now and keep it up for 6 months and wait about surgery? (Oh, wait, it says 6 non-consecutive months. Arrghh!)
I really don't know what to do. Any suggestions? What did y'all put on your sheet? Please help......

I have the same problem but I have Aetna HMO and I have collected all my data in the past five years from the doctors. I tried on my own with the weight loss, with no success because sometimes I would start that day and by evening blow it for I have forgotten to be good. But I have religously gone to the Wellness center to exercise and if I had cut back on my intake maybe I would have lost more. I have yo-yo diet for a while and my metabolism is stubborn. What doctor are you considering? I want to send my forms in soon to John J Scarborough, JR, MD, he is located on South Cobb Drive. Anyway I hope this has helped so I would suggest going to your PCP to monitor your weight loss for this surgery or the doctor you choose for the surgery. That's what they have been telling me.

The last dr monitored attempt I had was 10 years ago. However, when I had been diagnosed with diabetes in Feb 03, I began seeing the nutrionist at my endorcinologist office in case I needed a current 6 month program.
I was in month 4 when I had my first surgeon consultation. I have BCBS of GA HMO with POS option. The way my diet history looked was like this:
02/03 to 07/03 Suwanee, GA Endocrinolgist Nutritionist -38 lbs
regained 50 lbs
11/01 to 03/03 Doraville, GA Richard Simmons Deal a Meal -40 lbs
regained 20 lbs
04/94 to 12/94 Doraville, GA Medifast -100 lbs
regained 150 lbs
So I would just list any and every thing that you have ever tried.
