They have these at Officemax or Officedepot or Staples (you get the picture heh

) relatively inexpensive and you just fill them out and take them to the notary. hth, Michelle

Tonya, I got a ton of my forms free from the following website: http://www.familycaregiversonline.com/legal-medical.html#Power%20of%20Attorney%20Forms. All you need to do is fill in the blanks and get it notorized. Denise
