Hi, I work for the Newton County Schools District and have BCBS HMO. They require a letter of medical necessity from your doctor. A medical clearance letter, psychological and nutritional evaluation, proof of 6 month supervised diet, diet history. That's what they asked of me. Best of luck on your journey!! Chandra
