Denied by Aetna Today

I received a letter today stating that I was denied because of lack of documentation of a nutrition and exercise program and multidisiplinary surgical prepatory regimen (whatever that means). Now I am stomped. What are they looking for, does my PCP have to go to Weigh****chers meeting with me? She sent a summary letter stating all the diets that I have tried and failed. How specific does she need to be? *********HELP**********
