Description:

Support groups are a necessary part of your journey and well-being in this process.  If you are a NEW PATIENT, as part of your preparation for surgery we require proof of either:  2 live support group attendances (with signed verification form) OR 1 online group attendance (with participation printed and sent in with paperwork) and 1 live support group attendance.
