in need of some help

i was recently denied  because i didn't have some sort of print out stated how often i work out, the problem is that i work out in my apartments gym so there is not way for me to get any kind of print out, i was wondering if anyone on here can help me by sending me a copy of what it needs to look like, i can just type it myself, please if anyone can help it would be greatly appreciated

If you have Microsoft Word on your computer, you can use an included template to create calendars. You could use one of those and fill it out just like you would a to-do calendar.

your NUT or surgeon may be able to generate something for you. My surgeons office retrieved the info from my PCP b/c thats who I did my 6month supervised diet with. Also check your PCP, he or she may be willing to go back and put something together for you to present with your package.

I worked out at Curves as part of the approval process. As well, I looked online a printed out a calendar and kept up with the other activities outside of Curves. When it was time for me to submit my paperwork, the owner of Curves typed a letter stating my workout regimen and also the dates that I worked out. It was sweet, short and simple.

The insurance company wants to see where there was "supervised" workouts that can be proven. ie: fitness club, YMCA, personal trainer, etc.

I feel your pain and didn't realize this was the case until I was a month into my approval process. Luckily, I found out the real deal and was able to get it together.
