I have United Health Care, but all State employees are really insured by the State Health Benefit Plan and the different companies are just managers for the State.
I had to sign up with the nurse at the insurance company then have a 6 month physican supervised diet, release from a psycologists, and 6 months of support groups to satisfy the requirements of the insurance company.
Dr. Maciks office required appointments for cardiac, pulmonary, and upper GI clearances in order to make a date for surgery. I have those appointments this week and also have a surgery date of 11/16/09!
