well I called back and spoke to another case manager... she could not find any trace of my packet that was sent.. she gave me a new fax number and told me to have to dr's office fax it again.. i called the Ins. coordinator at the surgeon's office, Shay, and she immediately sent it again.. I will call in the morning to confirm they received it.. hopefully all will go well this time.
