Being charged an application/insurance filing fee
I know that some doctors charge 'program fee's but that is more around the classes they have you take.
Good luck! I know that Dr Procter and Dr Richard doesn't charge such a fee.

Let me tell ya how that $250 was a godsend for me.... I was in ins paperwork hell at Johnson/Hart's office... no fee but since they gave me erroneous info regarding my ins requirements I wound up paying over $500 in unecesary visits etc etc .....not to count the emotional/stress that I enountered...
I think this is a small price to pay for competence...... I now have a friend who is filing with Macik because she was going to a true results company who screwed up her paperwork badly tooo.....
You get what you pay for..... and you have to be financially ready for this surgery.... you will need to take vitamins, b12 shots, protein drinks etc...... This is vital and not without monetary investment into yourself......
