Peach State Healthcare what do they need?

Need to know who has peach state and what clearances or documents they specifically need. I have the list of clearances I need for the surgeon but does anyone know if pshc need anything else.

This is copied from my blog, it is everything I had to turn in to Peachtree Bariatrics for Peachstate to approve me.  Good Luck!

1) See your PCP for the following
    a) Letter of medical necessity AND medical clearance
    b) Bloodwork (make sure to include the H Pylori test)
    c) referrals for the cardiologist, psychiatrist, pulmonologist (if necessary),
         and nutritionist (if your insurance covers it) otherwise it's out of pocket, like mine
   d) physical and history


2) See cardiologist for clearance letter for surgery
My appointment is Wednesday, June 4th with Cardiology of GA, in Snellville.

3)  See psychiatrist of licensed psychologist for letter of clearance
My appointment is July 7th, with Dr. Gary Santavicca.

4) See nutritionist for letter of clearance.
Saw Alice Shuler, she was great.

5)  See Pulmologist for letter of clearance (if you have sleep apnea)
 I don't have to do this one. ( I thought I didn't need to do this, but just for safety, I have an appointment 7/28)

Diagnosis code must be 278.01 for medicaid/Peachstate

Ok, so basically the things that need peachtree bariatrics needs. They mentioned something about calling the insurance co to see if they needed anythings specifically. My pcp mentioned me going to the health dept for the nutrionist, I haven't called yet. How much did the nutrionist cost you?
I've got the letter from the psychiatrist. I've got an appointment for the cardiologist and pulmonologist next week.
Did you have to go to the pulmonologist? I do have sleep apnea. Did you have a specific medical reason to help you get approved?
What do you mean by diagnosis code explain that one please.
I know a million questions right? I am just so ready. Especially since I work at a Headstart so being out of work this summer is the perfect oppurtunity to get it done. I really want to have everything turned in by the end of April. Thanks so much. I'm sure I'll have many other questions.

I would like to know the same what do you mean by diagnosis code? Was the nutritionist expensive? Does Peachstate not cover that and did you  find a phycatrist that takes Peachstate?

I'm not sure about the diagnosis code. I was told to call your local health department for the nutionist. They are psychiatrist that do take medicaid go to the peachstate website and it should help you find one in your area.

I used to be a procedural coder for surgeons.  A diagnosis code is the code or codes which explains the reason for the procedure or office appointment, taken from a coding book called the ICD-9 book.. The procedure code is called a CPT 4 code. These codes are used for billing for you visit or also in the case of justifying your need for surgical authorizations. Hope that helps.

(I am living in Folsom,CA but our grand kids and my three Georgia peaches cousins live in the state. Trying to talk my husband into retiring to your beautiful state.)

Thank you  I must have had a blonde moment ...lol I went to school for med assisting you would  think I would have thought of ICD-9..lol .... Oh and I hate GA trying to leave the state...lol I am sure it is a lot cheaper then CA though I know some friends out there.

I don't think I looked hard enough for a nutritionist that accepted Peachstate.  I paid around 50 dollars for my nutritionist (Alice Schuller @ Dekalb Medical).  I also used Gary Santavicca for my psychiatric clearance, he accepts Peachstate.

I just got back here from Oregon. I have a hard time believing many places in Calif are cheap to live that are decent.

My husband works for the STATE OF CA and ole Arnold is stealing our pay and making him work cause he is a Psych nurse at a prison.  Big bummer! 

If my SCarolina beach city wasn't burning up so badly I might think of Myrtle Beach.  My step daughter is in Lawerenceville,GA.  She loves Savanna.  What do you think of that city?

The only thing I loved about savannah was Tybee Island I lived there for a year  the crime rate is higher there then Atlanta I lived  near  Hunter army air field. I just moved out of Lawrenceville to Norcross which isnt any better might I add...lol Lawrenceville is ok its busy  the crime in this general area is just getting bad, drugs , gangs ect... out of the two Lawrenceville is  better.
