SHBP for Unitedhealthcare members

It is terrible what Cigna is doing to it's members. As far as I know, UHC has not added any new requirements. They changed the counseling to where support groups could be used. As far as I am aware these are their requirements.
1. health assessment
2. 6 month supervised diet with doctor
3. 6 month support group meetings
4. psychological evaluation
5. Bmi of 40 or 35 with two co-morbidities
6. must go through optum health bariatric services (caseworker)
I hope this is all. If anyone knows anything diffferent please let me know.

I wish we had access to a caseworker. It would be comforting knowing you have one person assigned to you who you can contact for questions and updates.     Dealing with someone in a customer service call center means you get 10 diffreent answers to the same question depending on who answers the call.

 

 

I was told last night at a seminar that UHC is requiring 5 years of past weights.... like actual copies of the doctors note page with the date.  One weight for each year- 2004-2008. That may be a normal request- I don't know-  

Have not had surgery. My plan does not require 5 years of past weights, only 6 months of weight loss program.

Yes..l have UHC..not state employee..but UHC does require 5 yrs documentationn of obesity

HEY JESS, I HAVE SHBP AND I HAVE UHC AND I TALK TO MANY OF THE UHC MEMBERS OFTEN WHO HAPPEN TO BE STATE EMPLOYEE AND NONE OF US HAVE HEARD THAT. SO FAR UHC SHBP REQIREMENT ARE LISTED BY RAVEBHAVEN.
