Getting WLS with the State of GA health insurance

hey I was told by the surgeon's office to provide anything that could go towards my weight loss in the past...i sent a packet of weight loss attempt documnetation that I had kept over the years....some stuff they have included in my submital packet.

 

Then of course some stuff CIGNA or SHBP will just REQUIRE.

Thanks. I am leaving in a few minutes to go see my PCP and see if we can hammer it all out. I have appts. next week for the surgeon and nutritional counseling and then in two weeks for the psych. It may all happen sooner than I had hoped!!
