6 month diet

I am starting my six month diet in order to qualify for my wls. The nurse from the insurance company didn't mention any special paperwork that I needed other than the doctor's records, but I was wondering if anyone had suggestions for any records I might need to keep during my diet period to satisfy the insurance company? Also, I added an avatar pic, but it does not show up when I make a post. Any suggestions?

Read your insurance companys criteria for WLS carefully.
Here is what my CIGNA plan required. 

Active participation within the last two years in one physician-supervised weight-management program for a minimum of six months without significant gaps. The weight-management program must include monthly documentation of ALL of the following components:

Weight
Current dietary program
Physical Activity (e.g., exercise program)

Programs such as Weigh****chers, Jenny Craig and Optifast are acceptable alternatives if done in conjunction with physician supervision and detailed documentation of participation is available for review. For individuals with long-standing, morbid obesity, participation in a program within the last five years is sufficient if reasonable attendance in the weight-management program over an extended period of time of at least six months can be demonstrated. However, physician-supervised programs consisting exclusively of pharmacological management are not sufficient to meet this requirement.

All insurances are different and have different requirements.  I would suggest you keep a diary and make sure that in every dr appt you go to that you discuss diet and exercises.   
EVERY ONE and make sure your doctor documents that conversation including your weights.  Any nutritionist , diet program or dr that you have visited in the last 5-10 years you should get records from the more you have the stronger your case will be!  Good luck!

MOST IMPORRTANT..... even same insurance co's wil have different requirements based on your employees plan.........

It is employee plan specific...Verify your own plan's criteria... trust me on this one.. been there done that and denied.........
