Did insurance cover your sleeve? Which insurance was it?

I know there are all sorts of riders and cafeteria plans and whatnots, so just because Aetna covered you, doesnt mean they would cover me at my place of work - but I am looking for a place to start. 

My husband has serious misgivings about being self pay and something happening and complications not being covered and that complications will ruin us financially.  He wants to do all we can to see if insurance will cover it.   We have Cigna and they are not going to cover it - so I am seeing who else did have it covered and what insurance it was - to see if any of those are options for us.
