I thought your surgeon was supposed to assign one? Maybe try calling him/her. Probably would be good to use someone they have a history of working with because they would know what to document.

I had the same problem for months. Just out of the blue I thought to go to my Ins provider web page & look one up in their online provider page they had. I did find several but just to make sure I called the "customer care" # on the back of my Ins card & *POOF* they gave me a list of many that were "in network". Try that & see if either work for you.
