Has anyone with this insurance had gastric bypass surgery? I am missing 2 years from my 5 year weight history and I am looking for any suggestions. Please help.......
is the 1st record at least 5 yrs old? If so, you'll be fne. I was able to track down a dr that I saw 1 time and get a weight from '02 then I just had to get a record dated in '08 to be safe and I didn't have any problems.
