I have UHC with my employer and I was approved the first time it was submitted.  Best thing to do is look at your certificate booklet and see if there is an exclusion.  If you don't have a certificate book call the number on the back of your card and ask if WLS is a covered benefit.  Luckily for me the company I work for purchased the rider and it was covered.  The company I worked for prior to this one also had UHC and they had an exclusion.  So, it all depends on if your company purchased the WLS rider or not.  Best of luck to you.
