Need ideas for medical letter for pcp

I posted this on the main board but figured I'd post it here to. I had my consult with my surgeron last week. And was told that I needed a letter of medical necessity for my insurance. However my pcp is not that wls friendly. He said he thought it would be good for me, however he hasn't put any effort into the letters I've asked him to draw for the medical necessity previously. My surgeon said that I would need more than a 3 sentence letter to submit to my insurance for approval. So I was thinking it may just be better for me if I write the letter and take it into my pcp off so all they have to do is stamp it with his signature. Well, now I need some ideas on how to write this letter since I opened my big mouth...lol. Does anyone have a sample letter they used? Any ideas what needs to be stated in letter? Gurl I have them all.....I faught for 2 years! Be strong and never ever give up....Road blocks will come along but just push your way thru them honey.......its SO WORTH IT! I wil email them to you.
