letter of understanding

I have an appointment for my consultation, h-polaroid (sp?) test, nutritionist meeting, and the dreadful measurements and before pictures!!!! I have talked with the boss and we are gathering the necessary info to purchase a rider so insurance will cover my surgery. It will go into effect in November but I am willing to pay all the prerequisie stuff I can to keep the ball rolling. Anyway, I was wondering if anyone has an example of a letter of understanding. They will explain what they want from me when I go in for my appointment but I figure if I can go ahead and get that info and take the letter with me (!!!) maybe we can move right along. Shaniece
