Insurance Appeal Letter....PLEASE HELP!!

Ok I am finally in action mode. Unfortunately action mode just got me the answer I was dreading...Sorry ma'am you don't have coverage for that but you do have the right to appeal. WHAT THE HELL AM I SUPPOSED TO DO NOW?? Does anyone have a letter that I could use as an example or a reference site that might have some available?? I'm freaking out and worried! Shaniece :help:
