help!! please!! info papers for dr office?

i am filling out info for the dr's office and i don't know if i should fill it out as desparate needy or truthful? i have my appt for my consult appt tomorrow and i feel completely un prepared. my primary has been on vacation the past week and half so i wasn't able to call her and prepare.i have bcbs and i called the insurance company twice and they said it wasn't exculded in my policy but just medically necessary. has anybody had any luck with them this year? any help you can give i would love!! please..... thanks suree
