co-morbidities (referral letter)????
I need everyones help, Just recently I was reading some profiles at random and came across on with a very long, detailed, and interesting letter to the insurance company that she and her PCP had composed about all of her co-morbidities. I should have remembered the name but it doesn't come to mind. Please if anyone has something close to this that they are willing to share with me, I would really like some advise in the lay out. I have a list of co-morbidities, but finding a way to word them is getting me so frustated (sp?). I would love any and all advise. I may not be able to respond to your reply's until later tonight, have to spend some quality time with my dh and son's. But I will reply later tonight, I promise.
Thanks in advance...
Jerrie

Jerrie,
I don't know who you're referring to, but I do know there are sample letters in the "Your Journey" section (see the header above). I don't remember exactly where, but there are several kinds of letters. I used one of them as the "template" for my letter to all of my doctors. I asked each of them to write a letter about my co-morbidities as they pertained to his particular speciality (PCP, pulmonologist, orthopedics, etc.).
I hope this helps. Good luck on your journey. It can be a long process but it is soooo worth it!!
