I too was having the same problem. What I did was switch doctors. Call Dr Samuel F Cox in Ft. Lauderdale. 954 928-0066 He is a Medicare provider which means that he has to take whatever Medicare is willing to pay. Have not met him yet but others on the board have. His office manager Carey is awesome. He will lead you step by step through the process. Hope this helps. Kat
