Anyone been approved with Health First Health Plans?

I was approved through United Healthcare PPO for a Lap-Band however, I did not have the up-front money to get it done. Recently we changed insurers at work and I will have Health First beginning 1/1/05 which would reduce my OOP from 3k to about $300. Now, has anyone had success being approved? Thanks!
