My insurance didn't ask for the weigh in notes but I sent them instead. In most cases a PCP letter of a consecutive diet isn't good enough. Every time you went in to see your PCP, if it was for illness or weigh in, they always take your weight and write it at the top your the Dr's notes for that visit. You should have several sheets. I think mine was like 20 pages. If you call your PCP they will know what you are talking about. If you still have problems ..just request you medical records and let your surgeon's office pick out what they need. Hope that helps.

Hey there. I have Aetna HMO and what I did was get copies of all the office visit notes for tfive years. Along with that I also got copies of all the prescriptions that my pcp wrote for my comorbidities. And a letter from my pcp, psychiatrist and pulmonologist supporting the surgery.
