another medicaid/share of cost question?

I know, I know...........I have asked some many questions pertaining to this topic. I have another though. I want to know if someone who has used medicaid/share of cost can share with me a list of what is needed by them to get an approval. I want to know this prior to my consultation (which i hope to schedule this week), since I am paying for that out-of-pocket. I dont want to go to the consultation pay cash and then they say I need 12mths supervised dieting or something and I dont have that. Please help! Thanks to any and everyone *****plys. Michelle.

Hi!!! I am Faith. Figuring that there are people scrambling for surgeons who accept medicaid in florida I have 3 in south florida. Dr Breslaw 561 272 1234, Dr Fred Simon 561 798 8587 and I Dr Carlos Carrasquilla in Fort Lauderdale. I have Humana Medicaid and they stated that I sounded like this procedure was a medical necessity and they would approved based on what my personal symptoms. So keep up the hope. And harass your PCP's and Ins Companies TO DEATH or else it will be death for us. Love and Peace Faith
