Help! Looking for Psych clearance eval in Orlando

Anyone know who I can get to do the psych eval clearance in Orlando area? I called one that my PCP suggested but they didn't know what they should do. The surgeon just gave me a prescription form with "Psych evaluation, clearance for gastric bypass" written on it. Any suggestions from the board would be appreciated. Amy

Call Dr Adley Thebaud in sanford. He does them all the time, he has done 4 for people from my office alone. I do not have his number but he is behind central Fla regional hospital on 17-92. hope this helps. you can get his number from 411. Stephanie

Stephanie, Thanks for the name of the doctor in Sanford. I live in Longwood and work in Lake Mary so that would be so convenient. Fortunately, I got an appt with a psychologist in Ocala which works out because I have to go there for the mandatory nutrition class that same day. I was curious though, how much did Dr. Thebaud charge for your eval? I nurse said the psych evaluation typically isnt covered by insurance so to be prepared to pay $250. A small price to pay if I am approved but I was curious if that is a typical cost for the evaluation.
