Will NOT give written requirements of benefit.
I called my Insurance Humana Family -Florida Medicaid, to see if I could get what they told me in writing they said they don't do that. They said when it comes time for my paperwork to be submitted they will give them a consent form. I HAVE NO CLUE WHAT THAT MEANS.
Remember I told you guys earlier, that my insurance said I am covered 100% by medicaid, they will pay 100%. That my only criteria was:
Psych Eval
BMI of 40+
and 12 months diet history
Revision RNY 05/14/2014
Current Weight: 197
What does your surgeon's office say? And are they asking for a diet history or documentation of a medically supervised diet. Some want one, some want the other. At this point, what do you have to lose to get that stuff together and have your surgeon submit for approval. Lots of times the insurance companies work better with the surgeon's office than they do with the insured. I know that doesn't make sense but that is the way it is. I have a form that was in a surgeon's packet for a diet history. Send me a PM with your email address and I will send it to you if you like. Red
OK yes check now with the Surgeon's office you've decided to use. They'll be able to clue you in on what else you need as to what you already have. Best of everything will come your way soon.
