I just got off the phone with my insurance carrier - Humana Family - Florida Medicaid I wanted to make sure I knew what the requirments were. 1. Must have a BMI of 40+ ( mine is 44 something) 2. Must have psych Eval. 3. Must prove 12 months diet ..... said I can use weigh****chers record (QUESTION: not sure I have full year on my booklet, also my surgeons office wants me to do 6 months with nut. and PCP. I also have a log that i recoreded everything I did while I was on WW. what to do next?

You Need to call them again and ask for the Requirements in Writing. As in these past 5 years of my Journey I've never heard of an Insurance Company Accepting the Weigh****cher Records. They have only accepted Doctor Documentations in your personal medical records. Has you Primary Doctor written every month  of your weightloss attempts? I do not know specifically about Humana Medicaid, but I have seen some insurance carriers that only require a diet history which has nothing to back it up and I have seen others that accept things like WW and other program records.  It all depends on the insurance carrier.  But NOT all carriers require a supervised diet. Sounds like you are on your way.  I would verify the requirements with your surgeon however as each surgeon has their own protocols. Red
