Sample Letters?

I hear that it is a good idea to write a letter to submit with your authorization request. Where can I find some examples to get started? Ali

I would save the letter in case you need to appeal.  The letter could strengthen an appeal but the first submission is going to be decided on pretty much on if  WLS is a covered benefit and if you meet the criteria.   Red

Hi Sandy: I was reading thru some posts to see if I could find anything on Sample Letters and I show this post.  I am having trouble with my insurance Cigna as they have not approved by surgery as yet.  I am desperate to get this done and was wondering if you could please send me your letter since you got approved so quickly.   You can send it to [email protected].  I would greatly appreciate your help. Ali, I forgot the other letter is to your surgeon from your PCP and if you use Dr J as you are going to his office in July you will get a sample letter in the back of the insurance booklet you get.  Like I said before this booklet needs to be you bible so to speak because it is a place to keep track of what you have done and still need to do. If you chose not to use Dr Jawad I will send you the other letter as well in PM.  Sandy Wesley

Go to this link and there are alot of topics at the top of the page. There are sample letters and a guide to write your own. I used this site to write my letters and got approved! I hope it can help you!!
