You need to write them a letter requesting the specific reason for the denial. In your letter go point by point on the requirements, stating how you have met the requirement and what documentation you have sent to back it up. It is somewhat hard to write an appeal letter if you do not know what they are looking for. If they persist with not giving you a specific reason you should send a copy of their letter and any letters you have written to the state insurance commissioners office.

I would say this is a good thing because it is easily fixable. If they are asking for more detailed information -- they are still considering your case. This is way good! Call the insurance company and ask them if they have a format they are looking for and see if you can determine the kind of detail they are looking for. Then call your physician's office and tell them that the insurance company is looking for more details and give them any information that the insurance company gave you. Sometimes the physician just writes a letter stating the facts -- but sometimes the insurance company wants the actual case notes that the physician wrote up the day of the visit.
