Need sample of insurance letter

Im trying to assist a fellow patient who needs a sample letter of medical necessity for the PCP...can you share your letter with me to pass on? Thanks guys!! Hugs!

Ruthie, I know Jan Ocala used to have a sample letter on her profile. I am not sure if it was a medical necessity letter or her own letter to the insurance company but there may have been both. I have been thinking about this recently because my PCP did one of those little threee lines on a prescription pad letters which totally ticked me off because I gave him a paper with all the things that the surgeon needed included in it. I've gotten in the habit of writing letters for my doctors and just asking them to make any changes then signing them...thanks... That way I know what I needed and wanted said got said... Sarah
