Medically supervised diet??what r they looking for??

I have BCBS Florida State Employee PPO I know I have to go to my PCP for 6 mths for a medically supervised plan but does anyone know exactly what the plan should be? Can anyone on this insurance tell me what the plan their Dr. looked like? July will make month # 6 for me but he never specifically designated a plan...I have been weighing in and talking to him???? He did send letters in 2002 requesting the surgery but I had medicaid and they denied me for it........I was 285 then and BMI of 41......now I am 321 and BMI of 47. Any input would be great, tomorrow I have my psych. evaluation. Power of prayer is always welcome too : )

It should be a 1,200 calorie diet. The basic diets they have on hand at the office. You can also get your pcp to type this sample letter and get it signed by pcp. 05/01/06: Weight management Appointment: (your name) came into the office to discuss her obesity. We discussed various weight loss methods and we have decided to try a 1,200 calorie per day diet. She will keep a journal of what she eats and attempt to increase her obesity. We discussed various weight loss methods and we have decided to try a 1,200 calorie per day diet. She will keep a journal of what she eats and attempts to increase her exercise level. Her weight today was 332 pounds. She will return in one month for follow -up. 6/12/06: Weight Management Appointment: (your name) returned today for a follow up appointment. We discussed her progress and she stated she was having difficulty staying on the 1,200 calorie diet. She stated she is unable to exercise due to the pain in her legs and lower back. Her weight today is 334 pounds. She agreed to try harder to adhere to the diet in the next four weeks. She will return in one month for the follow- up. If she is still having difficulty we will explore other options. This documentation must continue for at least six consecutive months. Your primary care physician may keep you on the same program for the entire six months or can use multiple methods. As long as it is clearly documented with a date, weight , type of diet and program and a physician signature. I hope this helped. The insurance company likes to see how it was monitored.

Dear Margo, I have BC/BS of Alabama. I had to have a 6mo. supervised diet. I went to my dr. once a month and weighed in. I turned into him a sheet of all that I had eaten and all the excersize that I did. I even wrote down how much water I drank. I used a low carb diet for my diet. I would gain one month and lose one month. Each month my dr. would fax my information to my surgeon so they would have the information. Just remember that you have to go to your dr. 7 visits instead of just 6. If you have any questions I would love to help you in anyway. I have alot about my 6mo. diet on my profile. I have Cigna, and they are VERY picky about the 6 month diet. I couple of points I learned about the diet: - Doctors notes must have diet, exercise, and weight information EVERY month. - The diet does not have to be any certain plan (whatever the doctor decides). - Make sure your diet is a FULL 6 months. The first time you speak with your doctor about your diet plan is a consultation. The nexts month's visit is considered your FIRST documented month of dieting (I got denied for this little oversight). Good Luck, Jim

One note -- on the sample there was a period were there was more than 30 days between weigh-ins -- be very careful of things like this. I have read of instances where insurance companies have disallowed the whole six months for something silly like that. When they say they want you to have monthly weigh ins they mean monthly. I realize the sample was only an example for the wording and format but I thought I would point this out because it really could happen. Red
