SAD/Loop DS? Can someone please point me to the literature on this procedure?

I saw someone on the board who ahd a SAD/Loop DS asking for advice on the board today.

Anyone on the board who's a WLS geek like me, can you give me some links to literature on this please?

It's known as SADI-S.

Just google SADI DS and you'll get some good hits.

This one is good there are diagrams.

This is an experimental procedure and I have heard some very disturbing stories of people ending up with this who thought they were getting a DS.

I would be interested in links also as I just had the Loop DS.  I haven't been able to find anything on-line about the loop procedure.  The information about the surgery came directly from the surgeon.  I felt like this was the only choice for me because I have migraines and I need to take aspirin occasionally when I can't get rid of it with Imitrex only. The surgeon told me that I could occasionally take aspirin in an emergency but to avoid all NSAIDS when possible.   This is the only DS this surgeon performs.  I'm not familiar with the "SAD"/Loop phrase.

May I ask who was your surgeon?

 m1=23, M2=11, M3=4(had stall for 3wks), M4=6 M5=6 M6=6 M7=4 M8=3

I am a VSG to DS revison!

DSers can take NSAIDs as needed, in most cases. I've take Aleve 2-3 times a day, nearly EVERY day, since I was one month post-op from my DS over ten years ago. The ability to take NSAIDs as needed was one big reason I chose the DS in the first place.

I too would like to know who your surgeon is.

My surgeon is Matthew Metz in Colorado.  I would not recommend him.

I'm curious too, so I've done a bit of digging on the SADI/Loop recently.  From what I've read, while both surgeries involve a VSG, the SADI/Loop is really a very different surgery from the traditional Duodenal Switch in terms of intestinal configuration.

The SADI-S/Loop, or more fully, "Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy" is an experimental procedure which involves a Vertical Sleeve Gastrectomy paired with an end-to-side duodeno-ileal diversion (instead of the duodenal switch portion of the traditional DS).

Given that the SADI/Loop is a new surgery, the long term weight loss results and side effects are not yet known.  It is definitely simpler for the surgeon to perform the SADI than the traditional DS.

I haven't been able to find a published study that describes results or concerns beyond 3 years out and/or with a large sample size.  If anyone comes across one, please share!

There appear to only be two research articles on pub med:

http://www.ncbi.nlm.nih.gov/pubmed/20798995

http://www.ncbi.nlm.nih.gov/pubmed/24725654

I could only find an active clinical trial at one hospital in Madrid:

 http://clinicaltrials.gov/ct2/show/NCT01685177

Scholar.google.com has some limited articles as well.

I wonder if it would make sense for ObesityHelp to set up a separate SADI forum so that patients could compare notes.  I'm a bit concerned that advice from traditional DS veterans may not fully apply to SADI/Loop patient given the differences both in terms of the surgery itself and also the resulting anatomy. For instance, there was a recent question on post-SADI reflux and while traditional DS veterans may have experience with reflux, the causes and nature of reflux in a SADI patient would likely be very different than that experienced by a DS patient, given that in the SADI, there is no alimentary limb and bile is intentionally directed past the pyloric valve.  Also, there may be differences in medications that SADI vs. DS patients are able to use.

Hope this helps!

Hilary

The more I learn about the loop DS, I agree that they are completely different surgeries. Your information has been very helpful, Hilary.  Thanks!

Thanks.  How are you feeling?  Are you on a PPI or H2 Blocker for the reflux? One word of warning my doctors passed on to me is to avoid being on a PPI long term, if possible, so hopefully this will resolve quickly.  Hope it is going well!   Please stay hydrated!
