DS or RNY
I was set on having RNY (revision from Lap-Band to RNY) but my dr. has recommened the vertical sleeve. I have read that the sleeve is the first step to the duodenal switch, which I would like to have after researching all 3 options. Has you dr. recommended DS instead of RNY? What factors made you pick DS and are you happy with it?
I chose the DS because it has the highest percentage excess weight loss, best maintenance of that weight loss, and hghest rates of permanent resolution of almost all comorbidities of any bariatric surgery presently available. Maintenance is very important and often overlooked. RNY has a surprisingly and sadly high rate of significant weight regain, and often people don't get to a normal weight to start with (though most people do lose a lot the first year). The DS also has the most normal style of eating - no laundry list of forbidden foods, no (or rare) dumping, which is NOT a weight loss tool but really a potentially very nasty side effect that some people get with RNY, you can drink liquids with meals, you can use any cooking method because you don't have to worry about fat content, you can use either lean or fatty forms of protein. You can also take NSAIDs safely, which is forbidden for life with RNY. Lots of reasons, but the biggest reason for me was the extent and durability of the weight loss.
I had the DS 8 years ago and no regrets whatsoever.

I chose the DS because if the long term maintenance of weight loss. I also had a band and have been severely obese most of my adult life. My surgeon does both the RNY and DS - but when I asked him what he would recommend to his wife if she were in my shoes - he said "hands down the DS".
I would also say that if you can - just get the DS instead of starting with a sleeve and then needing a third surgery. But I am not a doc - that is opinion only. :)

I've chosen the DS over RNY because of what both posters above has said. When I started considering WLS last year I was convinced that RNY was for me. I chose a surgeon & got approved. My surgery was scheduled for 3/5/14. However in the process of getting approved I researched DS. Needless to say it wasn't a hard choice to make. Im now waiting for approval for the DS. My thoughts are I dont mind waiting for a 2nd approval for a lifetime of the DS .

What they said^^^^. Plus I worked with a whole bunch of people who had WLS back in the early 2000's. There were about 10 of us. Today there is one who still looks OK and normal for her age...me. And I was the only DS. I waited 2 years for a surgery date and the whole time they were telling me just get the RNY. Yep they were thin first. But I got to stay that way!
Think twice and cut once! You already KNOW that a restrictive only procedure is NOT your answer. Why would you do it again?
Yup, the sleeve is the first part of the Duodenal Switch. The restrictive part. But what keeps our weight off long term is the "switch" part, the malabsorption part. It's what makes fat our friend. Bacon, sausage, high fat dairy, heavy whipping cream in my coffee, butter on everything, is now healthy. We don't absorb 80% of the fat we eat. PERMANENTLY. Our malabsorption does not go away.
Do your research, go to DSfacts.com and compare the surgery sketches. Look at the DS Math, that's magical stuff. That's how we can eat 3,000 calories a day and not gain weight. The DS has the highest patient satisfaction rate of any weight loss surgery. It has the highest percentage of excess weight lost and kept off long term.
I wake up every morning (4+ years out) and pinch myself climbing out of bed, because I still can't believe I'm skinny now. I don't struggle at all to stay at my maintenance weight. It's been a dream come true for me.

I'm always grateful that my surgeon pointed me to the DS as having the best results for taking off more weight and keeping it off. Once I started to do some research I thought keeping the pyloric valve with the VSG was a big plus. I also knew that I was regimented enough to keep on top of my vits so that didn't bother me at all. There are ups and downs with any WLS. I'm certainly more interested in my bowels than I would have believed possible a year ago. I'm super happy with the surgery and the results.
