lab diagnosis codes

Please share what lab diagnosis codes you use for routine labs. I know Vitalady has a list of codes somewhere but I can't find it. My last labs were not covered but all my previous labs have been covered except for once when I ordered thyroid labs with no thyroid codes.

I believe those are part of a $4.99 pack for list of vitamins and codes, may include something else. But when i put it on Excel and gave to my GP with all the codes, he didn't bat an eye to write the lab order... someone correct me if I am wrong that this is a charge item...
