First Labs - 3 months

Hi everyone,

I'll be getting my first set of labs done tomorrow. In preparation, I am not taking any of my vitamins/supplements today (eek!!) and I will fast for 12 hours before the blood is drawn.

Thanks to some wonderful people here, I have a list of the lab tests I need to get. I also added some that my surgeon had on his list (although Dr. K. doesn't order labs until 6 months post-op.)

This is the list I'm giving my doctor tomorrow:

CBC with differential
Comprehensive Metabolic Panel
PreAlbumin
Iron Panel (TIBC, Ferritin, Iron Total)
LFT (Liver Function Test)
Vitamin B12
Vitamin A Level (plasma)
Folate
Vitamin D, 25-Hydroxy

Intact PTH
Phosphorus

Lipid Panel
Serum Magnesium
Serum Selenium

Thyroid Panel (T3 Uptake, T4 Free, and TSH)

Serum Vitamin B3
Vitamin B6 plasma
Serum Copper
Osteocalcin N-MID
Urinalysis

I saw a few on someone else's list on the RNY board that wasn't on the other lists I have seen. Can you guys tell me if these are necessary?

1. Bili-direct (I don't even know what this one is!)
2. Thiamine (B1)
3. Uric Acid

Uric acid is what they monitor to see if you need gout medicine. I don't think that is a standard test unless you have a history of gout. I'm on a drug called allopurinol to keep my uric acid low.

Looks like a pretty comprehensive list to me. I just had my 6 month labs and didn't have all of those. I forgot to argue for the B1 and B6.

If your insurance pays well and/or you've met your annual out of pocket I'd throw in the B-1 while getting the others done.  Your configuration probably doesn't need as we do but it wouldn't hurt to have an idea what your B-1, B-12 and B-6 numbers are.  Do you take any Bs or B Complex?

Thanks, I'll add it to my list.

Right now, I'm taking a sublingual B-12.

B-1, B-9, and B complex are on my shopping list. (I'm waiting for my labs to come back so I can order everything else I need at once.)

I think you have everything pretty well covered and I don't see a need for a urinalysis or the 3 extra things. Good luck. Most docs freak at this much but my surgeon does this many.

You may know this already,...lower is better on the labs for calcium, PTH and osteocalcin.

3 months PO isnt necessary, I would wait till 6 months PO, then 12 months, 18 months, 24 months, every year after unless there is an issue.....

remove lipid panel get a VAP

remove PT/PTT , PT is too insensitive to detect Vik K deficiency, (you already have Vit K and Osteocalcin, which are sensi tiveassays) PT  detects imminent bleed, PTT is not needed for a DSer

Thyroid panel is FT3, FT4, TSH,  -remove T3 Uptake, replace with FT3

add BSAP-bone specific alkaline phosphatase

add 24 hr urine for metabolic stone risk profile

unless you are diabetic, remove HgbA1C, that is not necessary
