Where can I find a lab spread sheet

I am 7 months out. I just had my second set of labs done. I want to put them on a spread sheet so I can list them here for the vets to take a look at for me.  Any suggestions? Is there a file somewhere that I am not aware of.  THANKS SO MUCH

I'll send you a message with my email. Send me an email and I'll attach my spreadsheet template and send it back to you. 

The formatting here on OH is bad and you'll have to type out your labs. You can also fill out the template I send you, you can send it back to me and I'll tell you what I think.

I got my spreadsheet here:  livingthedslife  dot  wordpress  dot  com/ds-resources/labs/.  I chopped it up cause I'm not really sure about OHs policy on posting links. Just remove the spaces and replace the (dot) with a (.). I like that this sheet has ranges included for reference.
