Dr. Info sheet on DS
I put this up on Google Docs. It's a shared document which I use whenever I see a new doc that has something to do with my health. Like a new Primary Doctor or something. It could also come in handy if you end up at the emergency room.
You can EDIT the document in areas like, who your doctor is and when you had the procedure and anywhere you may feel you need too.
This copy isn't generic. It's tailored for my situation. I highly reccomend editing it for you.
