Any ideas on appeals letters,
 
i need to tell The insurance why iam qualified to the ds have and have proof on suffering basically!

What you need to show is medical necessity, not suffering. Sadly, your insurer is not really concerned about your suffering. People here care about you. Your insurer doesn't.
     If you would like, you can pm me with your email address and I will forward to you an outline to get you started. You will also need to take into consideration any specific reasons they have stated for a denial and address them, so you need to know exactly why you were denied.
