Xpost PLEASE help. Has anyone had the micropouch with Dr James Sapala????

Yes I was thinking of getting a second opinion.  Just wasnt sure because I know Roslin is such a great doc and I trust what he says totally.

Gina, I think a second opinion would be helpful as well. Even if it only confirms what Dr. Roslin has said, at least you would know where you stand.
     My suspicion is that the micropouch is made so high on the stomach (really almost on the esophagus) that the vagus nerves have been cut, which would make the pyloric valve nonfunctional even if the stomach were put back together. If it isn't that, I think Dr. Roslin has some other good reason for his concerns. He is a very experienced DS surgeon and does do RNY to DS revisions, so I understand your trust in him also. I believe there is a good reason for his reluctance in your case, we just aren't sure what that reason is (and would very much like to know!).
      In general, I agree with everyone else around here that distal RNY and ERNY are not the greatest. However, in your case, these may be your only viable options, other than not having any kind of revision at all. This really is a heartbreaker, but reality is that not everyone gets a second chance.

I do know he said something about it being so high up.  Im thinking if he cant do it to try Dr.
Sapala for a revison.  I know he does revisions but I dont know what he could possibly do for me.  So my next step is to call his office after I get my results from my UGI.  I really appreciate everyones help!!!!!

To my way of thinking, Dr. Sapala and his "innovative" procedure are what messed you up. Why would you consider him for a revision? And I've never heard of him doing a DS, whether primary or as a revision. 
      Your best bet for a second opinion regarding revisin to DS, which after all is what you really want, would be one of the surgeons known for doing RNY to DS revisions, such as Dr. Keshishian or Dr. Rabkin in CA. If the only thing that can be done is to revise the small intestines to give you more malabsorption, Dr. Roslin can do that for you.
     You could contact either Dr. K or Dr. R via email and I bet they would answer you.

I would also reccomend a 2nd or 3rd opinion with Dr. Keshishian in California.  He has LOTS of experience with difficult RNY to DS revisions.  Please check into him as well.  Send him your endoscopy & UGI reports and films.

I understand it is a far distance to travel, but, please do not rule him out, OK?



BTW, I am going to be traveling from Alaska to California, if I ever get approved!!!!!  And plan B is to go down to Mexico, if need be.

Jamie, I have followed your story for quite a while, and see how much you yearn for a revision.  If your plan B is to go to Mexico, I think you should know of the result someone here who was specifically a RNY-to-DS revision had with Dr. Ungson.  She was disappointed to wake up still having a pouch, not a sleeve that sends food through the pylorus.  Basically, she got a BPD without DS.  I haven't seen her on the boards in a while, and I am sorry to not remember her user name.  Her avatar featured a pair of black and yellow striped flip-flops.  I imagine you would be devistated to have that happen, after all you have gone through, PLUS, then self-paying in Mexico only to not even get a full DS.

Devistated would be a very large understatement to wake up and learn I was not "fully" switched.
