For me, it's the use of DS as a tool to not regain the weight.  I've proven several times in my life that I can lose successfully, but I've always regained the weight.  With the DS as a support tool, I look forward to eventually getting past this annoying intital post-surgery stage and losing the weight without having to constantly count every calorie I'm putting in my mouth because I'm afraid I'm going to gain it back again.

If there is any possible way you can lose the weight and keep it off without surgery - DO IT!

The surgery will leave you with mal-absorption of vitamins and minerals for the rest of your life.  You could have serious health issues from the surgery itself, as well as from the mal-absorption afterwards.  Hernias, blood clots, bowel obstructions, osteopenia, osteoporosis, vitamin deficiencies, death...If there is any belief in your mind that you can take it off and keep it off without this surgery, you owe it to yourself to do it.

I couldn't...believe me, I tried.  And even if I could have taken it off, I still would have had diabetes, high blood pressure and high cholesterol.

If dieting worked long term, I would never have needed the DS.  Every time I dieted, I was SURE it would be the last time and the cure.  And every time, I regained plus more.  My DS is like a dream that I'm not waking up from.  Today I ate out at Red Lobster and ordered Crab Linguine Alfredo and ate everything but the linguine pasta.  If I had dieted my weight down and tried that, it would be the start of another back-slide, because I would have eaten the cheddar biscuits (all of them), and dessert besides.  With my DS, I have to eat protein first.  After I eat my protein, I don't have room (and no appetite for) anything else, dessert included.

Being incapable of developing diabetes with altered intestines is a huge benefit, I think.

I'm very young, had the DS because I was on my way to diabetes, and I'm SO relieved that I don't have to worry about that ever again.

Seriously.  Peace of mind.

 You'll never have high cholesterol post DS either.  Regardless of how much fat you eat.

 Not an answer to your question, but a point to ponder...have you had a sleep study?  One of the comorbidities most people don't know they have is sleep apnea...and that kills people far more often that we think, I'm sure.

Answer...I would think that losing weight without any surgery is ideal, because imposing malabsorption for the rest of your life has its costs.  But one of the offsetting benefits is the amount of weight loss and the duration of loss...and in those arenas, DS leaves diet and exercise in the dust.

There is definitely a metabolic effect with the DS.  I left the hospital off my diabetes meds.   A year later, almost all my comorbids and all of my meds are gone.   BP normal 120/80 no meds;  Diabetes resolved HgA1C 4.8 (low end of normal) no meds no dieting;  cholesterol - 103, no meds, no dieting.

If you can lose the weight without surgery, well good luck.  Of course, why did it get so bad in the first place or are you just in denial?   Statistically, your chances of losing it are in the low single digits and even lower for keeping it off.

Those who lose 100+ lbs with diet/exercise stand a 97% chance of gaining it all back.
Those with a DS don't suffer the same through the diet, because grehlin is not being produced as much.
Those with a DS will usually have an instant remission of Type 2 diabetes, while those doing diet/exercise will eventually see changes down the road, and it will all come back when they regain.

  Had the surgery with no comorbidities, just the weight to lose.  Had the surgery with the hope of not ever gaining that weight back.
