You are right to add back the vitamin C, I don't know why the NUT told you to cut it out. You need to bump up your B12 alot, 400's is way too low. Your old level of 1500 was perfect, that is what mine is and most people are jealous. I would go to a doctor and get an injection to boost it up quickly. If it gets much lower you can get permanent nerve damage. Idealy you should get injections twice a month for a few months until you get the level back to normal.
The only other thing I see is your vitamin D is in the toilet. 23 is way too low. Although your PTH and calcium are good, the D is way too low. You want your D to be at or close to 80. I know many people have a hard time bringing it up, but you should increase your D3 to get it higher.
